This form is for past due certifications for any MCHs that did not recertify for 2026 and is due on or before October 31, 2026.

Please note, both this past due recertification form (with credits earned in 2025) and the current recertification form (with credits earned in 2026) must be submitted to maintain your certification in 2027. Questions? Contact MNLA.
